Event Registration

*This event is for Veterans to go fishing. In the event your family is invited to this event, please understand they will NOT be able to fish on the boats with you. Please feel free to bring your own fishing gear if you wish to shore fish or rent a Shelter Cove rental boat following this event.

If you have electrical needs for medical equipment please bring a 50 foot extension cord. All shared camp sites have electricity.

TAVF will supply Friday evening BBQ dinner, Saturday morning breakfast and lunch at no cost to you and your guests. Shelter Cove Resort has a complete café and store well supplied.

TAVF will also make available to shared camp sites, firewood, upon request by our Veterans.

Oregon boating law requires every boat operator to have enough life preservers for every person aboard their boat. All boats shall adhere to the minimum safety requirements per the Oregon Marine Board for the State of Oregon.

The targeted fish on Odell Lake are Kokanee. Best results for catching is by trolling with long line or using downriggers. There are plenty of fisherman able and willing to assist new boat captains if you have any questions.

PLEASE NOTE: All boat assistants will have to register separately.

1. In Consideration for receiving permission to participate in the Take A Veteran Fishing At Odell Lake event (TAVF at Odell Lake), I hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE. Take A Veteran Fishing At Odell Lake, Vacations West Corp, Shelter Cove Resort & Marina, and within any State of the United States of America, their officers, agents, servants, or employees ( hereafter referred to as Releases) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any of the property belonging to me, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES, or otherwise, while participating in such activities, or while in, on or upon the premises where the activity is being conducted.
2. I am fully aware of the unusual risks involved and hazards connected with this activity, including but not limited to travel risks and/or, I hereby elect to voluntarily participate in said activity with full knowledge that said activities may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or any loss or damage of property owned by me, as a result of being engaged in such activities, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES OR OTHERWISE.
3. I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS the RELEASEES from any loss, liability, damage or costs, including court costs and attorney fees, that they may incur due to my participation in said activity, WHETHER CAUSED BY NEGLIGENCE OF RELEASEES or OTHERWISE.
4. I understand that Take A Veteran Fishing At Odell Lake carries minimal insurance covering any circumstances arising from my participation in this event or any activities associated with or facilitating that participation. As such, I am aware that I should review my personal insurance portfolio.
5. It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the United States of America.
6. I give permission to Take A Veteran Fishing At Odell Lake Salem, Oregon Chapter staff or personnel, to seek reasonable and necessary medical treatment for participants during such event or associated activities, and agrees to be responsible for any cost of such treatment.
7. I am at least eighteen (18) years of age and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by same. If participant is under the age of eighteen (18), Parent/ Guardian consents to the minor’s participation in the event.
8. IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it, and sign it voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made.

Form Instructions

You are completing this form to be a participating Veteran, Boat Captain, Boat Assistant or Volunteer of Take A Veteran Fishing on Odell Lake event. As such, you are certifying that you are or have served as a member of the United States armed services ( Army, Air Force, Navy, Marines, Coast Guard or National Guard) and you have a valid Military ID card or a DD 214. If you are signing up as a volunteer Boat Captain, Boat Assistant or Volunteer, you are certifying you are able to assist the event coordinator with tasks that you have signed up for without compensation. Everyone completing this form shall be contacted within 48 hours after submitting this form.

Please fill out all spaces which relate directly to you. Be sure your contact information is accurate. Until you receive a confirmation from the TAVF coordinator either by email or phone YOU ARE NOT REGISTERED. If you do not hear back from TAVF after 48 hours please contact us to locate your registration. If TAVF does not hear from you or send you a registration confirmation you are not registered.

If you have been confirmed by the event coordinator as a participating Veteran, Boat Captain, assistant or volunteer and you later cannot attend, PLEASE, contact the event coordinator as early as possible to cancel your participation. There may be a waiting list of fellow Veterans whom would like to participate to fill your position.
